What? MGO is seeking your assistance by asking you to complete a short survey.
Why? For two years MGO has had a business opportunity to assist OhioHealth in the development of their ambulatory strategy. A group of MGO physicians has served as consultants to their process. By you participating in the survey MGO can provide OhioHealth with a much broader base of physician opinions. Overall the survey results will assist MGO in fulfilling its commitment to provide valuable consulting services to OhioHealth.
The survey typically takes less than 10 minutes to complete. The questions seek to create an understanding of things such as what influences your decisions on where to refer patients, existing referral patterns, and satisfaction with a variety of OhioHealth’s services.
Who? The survey should be completed by a physician or key staff person in the office.
What’s in it for you? - Incentives:
The first 50 participants to complete the survey get a $10 restaurant gift card
All participants are placed into a random gift drawing for:
$50 Cameron Mitchell gift card
Family membership to Zoo or COSI
Lunch for your staff (up to $100)

When? The survey will be open through October 8, 2008.
Other: Although you will have the opportunity to provide your name or email address to be entered into the prize drawings, your personal data will be kept completely confidential. Your name and email address will not be tied to your survey results or be used to contact you in any other way.
